The best senior footwear rarely comes down to one feature.
Fit, fixation, heel height, collar support, slip resistance, and sole hardness each help on their own, but the most telling research looks at what happens when several are combined. Four case studies stand out.
Standard shoes beat slippers and heels, every time
A gait study of 57 older women compared barefoot walking, backless slippers, high heels (3.5cm and above), and a standard shoe with lace fixation, a low rounded heel under 2.5cm, an EVA midsole, a 6.5cm heel collar, and a treaded sole.
The standard shoe consistently produced faster walking speed and longer stride length than every other option, across single and dual-task walking conditions.
No single feature did this. The combination did.
Enclosed footwear outperforms open designs
Another study compared backless slippers to enclosed slippers with Velcro fixation, a hard rubber sole and a solid heel counter.
The enclosed design gave better directional control, less postural sway, and a narrower step width. Participants also rated it more comfortable.
Open, unfixed footwear gave up stability on every measure tested.
A purpose-built prototype performed best of all
Researchers went a step further and built a prototype shoe combining lace and Velcro fixation, a high collar, a firm heel counter, a 10-degree heel bevel for slip resistance, a defined tread pattern, and a textured EVA insole.
Compared to regular outdoor and flexible control shoes, the prototype produced a narrower step width and less end-of-step sway, both markers linked to lower fall risk.
Purpose-built balance shoes go further still
A separate line of research tested shoes designed specifically for stability: a longitudinal heel track, raised bands in the midsole, a metal shank for rigidity, and an arc-shaped tread.
These shoes significantly improved leg stability, especially with eyes closed, a harder balance test that removes visual feedback.
Wearers reported feeling steadier and more secure.
